Rancho Cordova Soccer Club Coach's Application (View)
CYSA Team Official Registration and Risk Management Disclosure Form (Link) Note: The Club form must be printed first then filled out. The CYSA form must be completed electronically prior to printing the forms. Please follow all the directions given on the site. Once the forms are completed attach a 1 by 1 inch picture (no hats or sun glasses) to the forms and mail them to the club. Club mailing address: Rancho Cordova Soccer Club P.O. Box 2741 Rancho Cordova, CA 95741-2741 Fingerprinting All coaches and assistant coaches need to be fingerprinted. If you were fingerprinted last year by the soccer club you do not need to do this again. This is for any new coach or assistant or anyone who did not do this last year.
